One example is that you may receive a call from an individual, and that person may claim that they are calling from Rushmore. They may know your name, and they may even know some information about your loan. They may tell you, for example, that your home is going to be sold at a foreclosure sale, and that the “only” way to stop it would be to pay this individual money, so that they can “stop the foreclosure.”
You should NEVER pay money to anyone unless you have confirmed that it is actually your loan servicer, and unless you are sure how the money you are paying will be used.
